Job Requirements:
- Assess clients needs, strengths, and challenges to develop individualized care plans and interventions.
- Provide counseling, support, and advocacy services to individuals, families, and communities facing social, emotional, or behavioral issues.
- Connect clients with appropriate resources, services, and support networks to address their needs and improve their well-being.
- Collaborate with other professionals, agencies, and community organizations to coordinate care and services for clients.
- Maintain accurate and confidential records of client assessments, interventions, and progress.
Job Requirements:
- Bachelors or Masters degree in Social Work or related field.
- State licensure or certification as a social worker required.
- Previous experience working in a social work or human services setting.
- Strong interpersonal and communication skills.
- Empathy, compassion, and cultural competence.
- Ability to work independently and as part of a multidisciplinary team.
