Responsibilities:
- Conduct assessments and create care plans for patients in their homes.
- Administer medications and perform treatments as prescribed by the patient's physician.
- Monitor and document patient vital signs, symptoms, and reactions to medications or treatments.
- Educate patients and their families on their medical conditions, medications, and treatments.
- Coordinate care with other healthcare providers, including physicians, therapists, and social workers.
- Communicate regularly with the patient's physician to ensure continuity of care.
- Maintain accurate patient records and documentation.
- Provide emotional support to patients and their families.
