The ideal candidate will be responsible for providing care to patients while adhering to compliance standards. This candidate should be able to recognize patient needs and prioritize those needs with the needs outlined by physicians. Responsibilities - Coordinates home care nursing with other members of the health care team. - Updates care plan as needed and as patient\xe2\x80\x99s condition changes. - Documents the patient\xe2\x80\x99s condition accurately and concisely, using correct charting format: in electronic medical record, notifies the physician of condition changes. - Observes and assesses patient\xe2\x80\x99s physical functions and psychosocial needs upon receiving to the home care programs - Identifies patient problems, symptoms and behavioral changes. Qualifications - Must be a Registered Nurse holding DOH/DHA/MOH License or is a DOH Passer/Eligible - Graduate of Bachelor of Science in Nursing or Equivalent - Newly graduated candidates are welcome to apply - BLS Certification - Has own visa / Can provide for own visa Job Types: Full-time, Contract, Permanent
