• Assist the Chief of Pharmacy in providing pharmaceutical services to include overseeing the safe medication management and use program within the inpatient settings, advising physicians on matters pertaining to drug usage and control within assigned hospital locations; participate in the development and delivery of continuing education programs for Pharmacy, Patients, Medical Staff and Other Healthcare Providers who are authorized to use medication
• Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses.
• Check patient medication cassettes prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed.
• Review and monitors drug therapy that requires dosing adjustments as authorized by the physician.
• Prepare IV admixtures and extemporaneous parenteral products under aseptic conditions.
• Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physician's prescriptions; ensure compliance with MOH laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.
• Mentor pharmacists in training; activities should include both practical and didactic experiences.
• Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, MOH regulations regarding drug controls, and JCIA standards.
• Conduct, verify and evaluate medication histories and inpatient prescriptions,assess compliance and suggest modifications so as to achieve desired outcomes.
• Instruct patients in the proper use of prescribed drugs; participate in patient care rounds with multidisciplinary teams to evaluate patient progress as needed.
• Individualize medication regimens using sound principles accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.
• Coordinate and developing the provision of patient-centered clinical pharmacy services across the hospital.
• Provide professional guidance to colleagues.
• Optimize the impact of the clinical pharmacy service within the available resources.
• Audit the medication use process in the inpatient setting to assure its meeting set policies, and procedures, MOH regulations and JCIA standards.
• Audit, organizing and do the monthly checking of medicines in crash trolley in the all the wards in the hospital.
• Anticipate and responding to changing requirements for clinical pharmacy services within the hospital.
• Promote/reinforcing and implementing the decisions made by the hospital's Pharmacy and Therapeutic Committee.
• Provide clinical review for medication revised by PTC comity.
• Risk management of medicine use e.g. active management of drug errors.
• Proactive develop and implement protocols and guidelines, in collaboration with other professionals, for the safe, effective and economic use of medicines in the inpatient setting.
• Support the provision of medicine information services within the hospital.
• Support the work of the Pharmacy & Therapeutic Committee
• Keep up to date with all issues regarding medication and ensure its effective dissemination of information throughout the hospital.
• Carry out any other duties that may be allocated by the Chief of Pharmacy to ensure effective running of the pharmaceutical service.
• Work within agreed Hospital and departmental policies and procedures and to accepted standards of practice.
• Comply with the code of ethics of the hospital and MOH.
• Bachelor of pharmacy degree and Master Degree in clinical pharmacy
• 2 years as a clinical pharmacist
• Extensive knowledge of drug products, medical therapeutics, pharmacokinetics and disease states
• Ability to demonstrate an excellent knowledge in pharmacy field.
• Excellent communication skills with good command in writing and speaking in English and knowledge in Arabic preferred.
• Follow specific instructions with high degree of accuracy.
• Excellent computer skills.
• Valid MOH License
