The post holder will work collaboratively with other team members, will uphold and live the NMC Nursing Strategy in order to provide safe and outstanding care to their patient, relatives, friends and significant others all the time, every time.Responsibilities:Assistant Nurse will:
Under the supervision of the Registered Nurse, assess and implement
nursing care for patients ensuring a patient centered approach to care.
Under the direction of the Registered Nurse, assist in the activities of
daily living for patients in accordance with the patient care plan e.g. distribution of patient meals and assisting patients with hygiene needs.
Maintain a high standard of nursing care for patients, working within
evidence based guidelines and unit specific competencies.
Demonstrate service excellence and act as a patient advocate to support
the patients' dignity and rights at all times.
Understand the basic indications for medications, recognizes side effects
and reports to the Registered Nurse appropriately.
Ensure the Registered Nurse is informed of observations and care
provided for patients and document appropriately.
Report any significant changes in the patient's condition to a Registered
Nurse immediately.
Respond to all levels of emergencies according to the organization's
policies and procedures
Facilitate post discharge clinic visits and/or tests as required including
scheduling follow-up and referral appointments
Help arrange patient transportation as required.
Maintain flow of documents for scanning if needed.
Give direct patient care according to the therapeutic plan and individual
patient needs as directed be the patient care plan under direction of a Registered Nurse.
Provide appropriate information to assist the Registered Nurse in the
preparation and maintenance of an effective patient care plan.
Record in the patient's official record a detailed report of care given
which reflects the therapeutic plan and specific aspects of the patient care plan under direction of Registered Nurse.
Reinforce instructions given to the patient by the physician and
Registered Nurse and refers new needs for instruction to the Registered Nurse.
Notify the Registered Nurse of significant findings or changes in the
patient's physical or mental conditions.
Initiate emergency assistance as per policies and procedures.
Perform other duties as assigned by the Senior Assistant Nurse or
Registered Nurse and within the scope of practice
Ask questions when in doubt for clarification by the Registered Nurse
Escalate to and seek expert advice from colleagues when unable to or
not competent to undertake required care. OSHAD:
Comply with all OSH and infection control policies, standards and
procedures and cooperate with hospital management to comply with those requirements.
Work accordance with the documented OSH procedures and
instructions, specific responsibilities
Be familiar with emergency and evacuation procedures.
Notifying OSH Hazards, incidents, Near misses and issues and
assistance with the preparation of risk assessments, incident reports.
Comply with Waste management procedures and policies.
Attend applicable OSH/Infection control training programs, mock drills
and awareness programs.
Use of appropriate personal protective equipment and safety systems.
Qualifications:
Have an appropriate license to work as a nurse in the relevant regulatory
health authority (DoH, DHA) with all the qualification and experience this mandates.
Manage their own professional revalidation
Have basic life support qualification (within past 24 months)
Have an active interest in the clinical field in which they are employed
Be willing to work across departments when necessary
Be able to communicate clearly and effectively in English (spoken and
written) DESIRABLE: The post holder should have:
Evidenced experience/expertise in the clinical field in which they are
employed
Evidenced ability to work as part of a multi-disciplinary team
Organizational and administrative skills with which to use the HIS platform
